There is something like a sandpaper sleeve, but instead it is metal with finely-ground industrial diamonds. It can cut ceramic as long as it's kept wet, just like the tile saw blade.

I made a cardboard template shaped to fit around one side of the door frame, and used it to mark the tile with a permanent marker (which isn't very permanent, actually). Then cut roughly with the tile saw; then cut the rounded areas with the Dremel.

I immerse the edge of the tile in water for Dremeling. Of course only the cutting bit should get wet, not the chuck or the rest of the tool.

I used a combination of pavers and such to fill the big gaps, and filled the small ones with mortar. Cut an extra piece of backer-board to make up the thickness difference, since the walls all have a thick coat of mortar over the block.
